TECNIS PureSee IOL in Brisbane: A Patient Guide

If you’ve started researching lens options before cataract surgery or refractive lens exchange, you’ll have worked out the central problem already: the more range of vision a lens promises, the more you tend to give up in quality of vision, particularly at night. The TECNIS PureSee, from Johnson & Johnson, is one of the more interesting recent attempts to soften that trade-off. Here’s what it is, what the studies actually show, and who I think it suits.

What the PureSee is

The PureSee is an extended depth of focus (EDOF) lens built on Johnson & Johnson’s TECNIS platform, a one-piece hydrophobic acrylic design with a long track record. It comes preloaded in an injector, and there’s a toric version for eyes with meaningful astigmatism.

The unusual part is how it stretches your range of vision. Most lenses in this space use diffractive rings that split incoming light between two or three focal points. The PureSee has no rings at all. The power of its back surface changes smoothly and continuously from the centre outwards, so light gets stretched through an extended zone of focus instead of being divided up.

Why that matters

Those diffractive rings are the main reason multifocal lenses can put halos and starbursts around headlights at night. Take the rings away and you remove most of that problem at the source. In a 2024 peer-reviewed study of quality of vision, roughly nine out of ten patients said glare, halos and starbursts bothered them little or not at all, and the worst “very bothersome” figure for any symptom was 3.4%. Contrast sensitivity, which is what night driving really depends on, measured much the same as a monofocal in the US clinical trial.

There’s a second advantage that doesn’t make the brochures. The lens forgives small measurement errors. In the US trial, 94% of eyes still saw 20/25 or better at distance even when the final result was up to half a dioptre off target. No lens power calculation is perfect, so a lens that shrugs off a small miss is worth having, especially in eyes where the measurements are harder to trust.

What range of vision to actually expect

Here’s what the data says, rather than the marketing:

  • Distance: excellent. This is where it behaves like a quality monofocal. In real-world results from 238 patients presented at ESCRS 2025, about 97% said they needed distance glasses never or only a little of the time.
  • Intermediate (computer, dashboard, cooking): very good. Around 95% reported little or no need for glasses at arm’s length.
  • Near (reading): useful but not complete. About 59% reported being free of reading glasses. Most people read a phone comfortably; a long session with a novel, small print or dim light will usually still want readers.

That near-vision number is the one to sit with before choosing this lens. In one study comparing it with an enhanced monofocal, the PureSee roughly halved dependence on reading glasses (36% versus 80% needing them). That’s a big step up from a monofocal, but it isn’t the full reading freedom a trifocal is chasing.

Who it suits, and who it doesn’t

The PureSee is the lens for the patient who tells me: “I want more freedom than a basic lens, but I’m not prepared to compromise my night vision.” Night drivers, people whose work depends on fine visual detail, and anyone naturally cautious about side effects tend to land here. Its tolerance of small refractive misses also makes it worth discussing when measurements are less predictable.

It isn’t the right lens if reading without glasses is your main goal. That conversation heads toward trifocal designs like the Alcon PanOptix Pro or full-range lenses like the enVista Envy, which buy you more near vision at the cost of some night-time light effects. And as with every premium lens, eyes with corneal disease, irregular astigmatism or keratoconus need proper individual assessment. That’s the corneal territory I work in every day, and it’s where honest advice matters most.
