enVista Envy IOL in Brisbane: A Patient Guide
Bausch + Lomb’s enVista Envy is one of the newest lenses in the “full range of vision” category, the group of lenses that aim to give you distance, computer and reading vision from a single optic. If you’re researching your lens options before cataract surgery or lens exchange, here’s what the Envy actually is, what the trials show, and where it fits.
What the Envy is
The Envy is a diffractive full-range-of-vision lens built on Bausch + Lomb’s enVista platform. The material itself has a real claim to fame: it’s the only IOL material whose regulatory labelling reports no glistenings of any grade. Glistenings are tiny fluid-filled specks that can build up inside some acrylic lenses over the years, and this one simply doesn’t form them. It also produces less colour fringing than many competing acrylics. There’s a toric version for astigmatism, and its range covers lower levels of astigmatism than most toric platforms bother with.
How it works
The optic uses what Bausch + Lomb calls an ActivSync design. The diffractive steps are taller in the centre of the lens and get progressively gentler toward the edge, which means the lens changes how it shares out light depending on your pupil size. In bright light, with a small pupil, light is spread across near, intermediate and distance. In the dark, as your pupil opens up, the lens progressively favours distance. That’s a deliberate choice aimed at night driving: give up some near range in the dark, when you’re rarely reading anyway, to protect distance clarity when it counts.
The transitions between those steps are also polished down to sub-micron heights to cut scattered light, which is the basis of the lens’s claim to a gentler halo profile than older trifocals.
What the published evidence shows
For such a new lens, the Envy has a solid evidence base. In the US pivotal randomised trial of 501 patients, published in the American Journal of Ophthalmology in 2025, it matched a monofocal for distance and clearly beat it for intermediate and near, delivering around four dioptres of continuous useful range. Vision held at the 6/7.5 level from arm’s length right down to reading distance, where the monofocal fell away sharply. About 89% of patients achieved 20/32 or better uncorrected at all distances. A separate Canadian randomised trial found 90% of Envy patients reporting little or no difficulty getting through their day without glasses.
On night vision, the fair summary is: better than the older diffractive lenses, but not invisible. In the US trial about 88% of patients were bothered little or not at all by glare and 91% by starbursts. For halos the figure was around 80%, which means one patient in five noticed more than a little. Severe symptoms were uncommon, 6% or fewer across the studies, and nobody needed the lens exchanged for optical reasons. Most people’s brains learn to filter the halos out over the first few months.
A note on the 2025 recall
If you research this lens properly, you’ll come across the March 2025 voluntary recall of certain enVista lots, so let me address it rather than hope you don’t find it. The problem was traced to a raw-material change from one supplier affecting specific production lots, which was linked to a rise in reports of treatable post-operative inflammation. Those lots were withdrawn worldwide, the cause was corrected, and supply resumed from mid-2025. The reported cases responded to treatment and no lens explantations were reported. Ask any surgeon offering this lens about it. The factual answer holds up well.
Who it suits, and who it doesn’t
The Envy is for motivated spectacle-independence seekers with healthy, regular eyes: people who read, work at screens and drive, and want to do all three with minimal glasses. The pupil-adaptive design makes it a reasonable conversation even if you do a bit of night driving, with the caveat above stated plainly.
It’s not the lens for eyes with significant corneal disease such as Fuchs’ dystrophy, keratoconus or scarring, nor for irregular astigmatism, significant dry eye or macular disease. Eyes that have had LASIK need careful individual assessment. As a corneal specialist, this is the part of the consultation I take most seriously, because the fastest way to create an unhappy premium-lens patient is to put a light-splitting optic into a cornea that can’t support it. One more technical point: with full-range lenses, even half a dioptre of leftover astigmatism measurably degrades the result, so precise astigmatism planning (and the toric version where needed) matters more here than with a monofocal.
Where it fits among your options
