Can a Cataract Grow Back? What Is Capsular Opacification?
It is one of the most common worries I hear in clinic: “My vision was perfect after my cataract surgery, and now it’s going misty again. Has my cataract grown back?” The short, reassuring answer: no — a cataract cannot grow back.
A cataract is a clouding of your eye’s natural lens, and during cataract surgery that lens is removed for good and replaced with a clear artificial one. An artificial lens can’t form a cataract, so once yours is gone, it is gone forever.
So what is going on when vision mists over months or years later? Almost always, the answer is something called posterior capsular opacification, PCO for short. You may also hear it called posterior capsular fibrosis (PCF) or, confusingly, a “secondary cataract”. The name is scary; the condition is not.
What is posterior capsular opacification?
When I remove a cataract, I deliberately keep the thin, clear membrane that wrapped around the natural lens, called the capsule, because it makes the perfect hammock to hold the new artificial lens in place.
PCO is a completely natural healing process. A few of your own cells remain on that capsule after surgery, and over time they can slowly spread across it, like a frost creeping over a window. The lens itself stays perfectly clear. The misting is in the membrane behind it, which is why the symptoms feel so much like the original cataract: hazy vision, glare around lights at night, colours losing their punch.
If this has happened to you, you haven’t done anything wrong, and nothing has failed. It’s simply your eye doing what healing tissue does.
Why do younger patients get it sooner?
PCO tends to appear faster in younger patients, which surprises almost everyone. The reason is almost flattering: younger cells are healthier and more energetic, so they spread across the capsule more quickly. In older patients those cells are far less active, so the capsule often stays clear for many more years.
So if you had your lenses replaced in your 40s or 50s and noticed misting within a year or two, that is simply your younger, healthier cells doing their job a little too enthusiastically.
The fix: YAG laser capsulotomy
A YAG laser capsulotomy is one of the most satisfying procedures I perform, because the improvement is so quick. A precise laser completely removes the scar tissue from the centre of the capsule. There are no incisions, no needles and no pain. It takes just a few minutes in the clinic, and most patients notice their crisp vision returning within a day.
Best of all, for the vast majority of patients the result is permanent. Once that central window has been cleared, the misting almost never returns, and one quick visit solves the problem for good.

Misty vision after cataract surgery? Let’s take a look
If your vision has slowly become hazy months or years after cataract or lens replacement surgery, don’t sit at home worrying that your cataract has returned. It hasn’t, and the fix is quick, painless and permanent.
Frequently Asked Questions
Can a cataract grow back after surgery?
No. A cataract is a clouding of your natural lens, and that lens is permanently removed during cataract surgery. The artificial lens that replaces it cannot form a cataract, so a cataract can never grow back or return.
What is posterior capsular opacification (PCO)?
PCO is a completely natural healing response in which the clear membrane that holds your new lens, called the capsule, slowly becomes frosted by your own cells. It is sometimes called a "secondary cataract", but it is not a cataract at all and it is easily treated.
Is YAG laser capsulotomy painful?
No. YAG laser capsulotomy is painless, takes only a few minutes in the clinic, and involves no incisions or needles. Most patients notice clearer vision within a day or so.
Can PCO come back after YAG laser treatment?
No. Once the clouded part of the capsule has been removed with the YAG laser, it does not grow back. For the vast majority of patients, one quick treatment restores clear vision permanently.
